- The facility's name and CDPH facility ID (shown on its page), if it's about a facility.
- The address (URL) of the page you're looking at.
- What looks wrong, and what you expected to see.
Disputes about a citation or survey
We can't change the agencies' records. Citations come from CDPH and federal survey findings from CMS, so a dispute about one has to go to CDPH. If our copy doesn't match theirs, that's ours to fix — please email us. See How our data works.
