ICFLookup is an independent, unofficial research tool for looking up licensed California Intermediate Care Facilities (ICFs). It is not operated by, affiliated with, sponsored by, or endorsed by the California Department of Public Health (CDPH), the California Health and Human Services Agency (CalHHS), the Centers for Medicare & Medicaid Services (CMS), or any other government agency. By using this site you agree to these terms.
2. The data is the agencies’, not ours
Every license detail, citation, penalty and survey finding shown here comes from CDPH and CMS public files and is reproduced as published. This site does not investigate, verify or editorialize on that record, and nothing here is its own accusation or finding about any facility, licensee, administrator or person. The data may be incomplete, out of date or wrong in the agencies’ own systems; confirm anything material with CDPH and the facility. Reorganized data shown here is not official government data.
3. Sensitive content
Federal survey findings describe the care of residents with developmental disabilities, sometimes including abuse, neglect, injuries and medical details. They are public records, shown as CMS published them, with residents identified only by number. This site adds no information identifying any resident.
4. Not advice
Nothing here is legal, medical, financial or placement advice, or a recommendation for or against any facility.
